If you have had a change of address, phone number or email address, we need written notification for our files to complete the change of address or phone number.
This must be sent to our Facility Office Address by United States Postal Service certified mail return receipt requested, or via a nationally recognized overnight carrier with signature confirmation to the Mailing Address; in person, or on Operator’s Website, on a form prescribed by Operator of any change in Occupant’s address, phone or email address. Call us at 508-398-3090 if you have questions. Calling or texting us does not constitute notice of a change of address.
